Health Quality Alberta, with the support of Assisted Living Alberta, will conduct a review focused on the care experience of long-stay Alternate Level of Care (ALC) patients who transitioned from acute care to community.
This review will focus on individuals who spent 100 or more days in acute care and transitioned between August 2025 and May 2026 to a Type A or Type B continuing care home or to a private residence with home and community care of any type and are an acute, long-term supportive, maintenance, rehabilitation, or wellness client.
The goal of this review is to better understand the quality and safety of care experienced during these transitions and to identify opportunities to strengthen care across the system.
To ensure a comprehensive and balanced understanding, Health Quality Alberta will gather insights from multiple perspectives, including:
- Continuing care home residents and clients who receive home and community care who are able to share their experiences.
- Family members most involved in Type A and Type B continuing care home resident’s care.
- Staff who supported and coordinated transitions.
- Existing survey data to compare experiences across resident and client groups.
- Administrative data to describe the sample demographics.
Findings from this review will be shared with Assisted Living Alberta and the Ministry of Assisted Living and Social Services to help inform future improvements to care and transitions.
